HDI offers comprehensive dental care for the entire family at its Wadsworth and Bayside locations. We also utilize our 2 mobile dental units to travel to schools for our "Smiles Are Everywhere" program that provides dental care to students. Dental services at our sites include x-rays, dentures, extractions, root canals, fluoride treatments, etc.

| The Department of Human Services |

The Department of Human Services and HDI have worked together to place an outstationed eligibility worker at some of HDI's medical sites. This person can help patients fill out Medicaid applications, determine eligibility for state and federal assistance, and answer questions.

| HIV/AIDS Services |

Hearth Home provides advocacy, educational programs, and social networking opportunities for People Living with HIV/AIDS; anonymous and confidential HIV testing and counseling; HIV prevention interventions for high risk populations; safer sex supplies and educational materials; relief pantry for People Living with HIV/AIDS. All services are free of charge.
The Janes Street Academic Community Health Center offers individualized direct care and medical services provided by a Board Certified Infectious Disease Physician and a registered nurse that are sensitive to the needs of people living with HIV/AIDS. All services provided by Health Delivery, Inc. are confidential.

| Mammography |

HDI offers a specialized mammography program at it's Janes Street Academic Community Health Center. Mammography is a specific kind of imaging that uses a low dose x-ray system for scanning breasts. Most medical experts agree that successful treatment of breast cancer is linked to early detection. Mammography plays a key part in early detection by showing changes in the breast before a patient can feel them. It is recommended that all women over the age of 40 receive a yearly mammogram. HDI offers free mammograms at it's Janes Street Academic Community Health Center to those that qualify and offers very flexible scheduling.

HDI also operates a migrant health program. We have seasonal migrant health sites that are open during the growing season. HDI provides medical and dental care in the communities that the patients work in. HDI ensures that bilingual staff is available to fully meet the needs of the farm workers. We make sure that this under-served population does not get neglected.

This program provides outreach to pregnant women and children. A team of health professionals work with patients to promote healthier pregnancy and healthier babies. The staff includes registered nurses, dietitians, and social workers. Services include family planning, childbirth education, parenting education, nutritional counseling, etc.

Health Delivery's Gynecologists provide care for medical and surgical conditions of the reproductive tract of non-pregnant women. The physicians are experienced in providing a wide range of services including annual exams, Pap smears and birth control counseling. Gynecologists maintain expertise in the evaluation and treatment of such conditions as Abnormal Pap smears, Menstruation disorders, Urinary incontinence, Pelvic prolapse, and Menopausal disorders.

HDI offers pharmacy services at our Janes Street location and at our Bayside location. Our pharmacy allows HDI patients to receive prescriptions at discounted rates up to 70% off.

Health Delivery Inc. may provide transportation to and from our health centers for patients. 48 hour notice is preferred. For more information, call 989-755-2053.

| Women Infant and Children (WIC) |

The Woman, Infant, and Children nutrition program is designed to ensure that infants and children up to age 5 and pregnant and nursing women receive the proper nutrition. Project Fresh allows you to buy fresh fruits and vegetables at local farmers markets during the summer.

HDI offers x-ray services at most of its sites. This is a convenient, timely way to get on site diagnostic radiology. All x-rays are performed by registered radiographic technologists and are read by a licensed Michigan radiologist.
